What is the Assistive Technology and Home Modifications (AT-HM) Scheme?
If you’ve ever needed help moving around your home or using everyday items more easily, assistive technology or home modifications might be just what you need.
As part of the new Support at Home (SAH) program, the Assistive Technology and Home Modifications (AT-HM) Scheme provides separate funding for products, equipment and home changes that help you live more safely and independently.
How does it work?
You’ll be assessed for the AT-HM Scheme during your aged care assessment. If you’re eligible, the approval and funding tier will be outlined in your Notice of Decision and Support Plan.
What can AT-HM funding cover?
AT-HM funding can be used for a range of supports, including:
- Assistive technology products and equipment
- Home modifications
- Clinical prescription of items
- Wrap-around services (such as training or setup)
- Coordination costs.

What’s included in the AT-HM List?
The AT-HM List outlines the types of items and modifications that may be covered. These fall into the following categories:
- Managing body functions – such as pressure cushions, anti-oedema stockings and memory aids
- Self-care products – including adaptive clothing, assistive products for toileting, bathing or showering
- Mobility products – like walking frames, wheelchairs and lifting devices
- Domestic life aids – such as assistive kitchen tools, drinking cups or cleaning supports
- Communication tools – including reading aids, writing supports and AAC (alternative and augmentative communication) devices
- Home modifications – such as grab rails, accessible showers, fixed ramps or safety barriers.
