VMCH Disability Services is a registered support coordination provider for the NDIS.

What is support coordination?

Support coordinators can help individuals with an approved NDIS plan exercise choice and control to achieve their goals. Our support coordinators can help people connect to service providers who offer the best options matched to each person’s individual needs.

How do I decide if I need a support coordinator?

A support coordinator can play a role in helping individuals navigate the often complex and time-consuming task of managing an NDIS plan for themselves or a loved one.

This may be an appealing option for some people. Others may feel more comfortable managing their own plan.

What do I need to do to ensure support coordination is funded in my NDIS plan?

It is important that if you are interested in including support coordination in your NDIS plan to raise this when you have your planning meeting.

What can a support coordinator help me with?

A VMCH support coordinator can assist NDIS participants with several tasks including:

Discuss what services will help individuals achieve their NDIS goals
Link NDIS participants to community, mainstream and government services
Negotiate with providers about what they will offer and how much it will cost
Ensure service agreements and service bookings are completed
Assist in preparing for plan reviews.
